Manhunt has 20 scenes, plus 4 unlockable bonus scenes. In Carcer City, death row inmate James Earl Cash survives a faked execution and is forced by Lionel Starkweather, "the Director", to kill gangs of Hunters on camera. After the Director turns on him, a journalist helps Cash escape, and the last scene, Deliverance, ends with Cash killing Starkweather in his mansion.

Cash is executed by lethal injection, but he was only sedated: Lionel Starkweather arranged it. Through an earpiece, the Director promises Cash his freedom if he kills the Hunters waiting in areas covered by CCTV. Cash goes through the Hoods, the Skinz, the Wardogs, the Innocentz and the Smileys, and Starkweather's Cerberus unit moves him between locations.
In Kill the Rabbit, the Director tries to have Cash killed as the climax of his film, but Cash survives. After killing Ramirez and the last Wardogs, he is picked up by a journalist who reveals the Director's identity and needs him to reach her evidence. Starkweather sends the corrupt police, under chief Gary Schaffer, after them; the journalist gets away with the evidence while Cash is chased through the subway and a train yard.


The Cerberus take Cash to Starkweather's mansion. Piggsy, kept chained inside, breaks loose, and Cash kills him before killing the remaining Cerberus and Starkweather himself. The journalist exposes the snuff film ring and the police's part in it, and Schaffer faces prosecution.
Manhunt was banned in New Zealand in December 2003 and, after first being rated, effectively banned in Australia in September 2004. A Munich court ordered copies confiscated in Germany in July 2004, and Ontario restricted it to adults. In the UK, media linked the game to the 2004 murder of teenager Stefan Pakeerah; police rejected any link and said the copy found belonged to the victim.
