Manhunt 2 follows Daniel Lamb, who escapes the Dixmor asylum with the help of Leo Kasper in 2007, with flashbacks to 2001. Hunted by the agents of a secret program called the Project, Daniel learns that he was one of its scientists and that Leo is an assassin personality implanted in his brain. The game ends with a battle between the two inside Daniel's mind, with a different outcome in the alternate ending.

The IGN guide and the Manhunt Wiki give episodes 1 to 14 in the same order. The Manhunt Wiki adds a 15th, Personality Clash, the ending inside Daniel's mind, and a bonus episode, Release Therapy.

A thunderstorm knocks out security at the Dixmor asylum and the cells open. Daniel, who has lost his memory, escapes with Leo Kasper, who guides him to the ruins of his home to recover medication. Bounty hunters and agents of the Project hunt them across the city of Cottonmouth.
Daniel was a top scientist on the Pickman Project, which created the Pickman Bridge, an implant meant to hold an assassin's personality. He volunteered to test it to pay off his family's debts. The Bridge malfunctioned: the implanted personality became Leo, whom Daniel sees as a real man. Under Leo's control, Daniel killed his wife; the Project erased his memory and locked him in Dixmor. Dr. Laura Whyte reveals the truth after Daniel is captured.

In the standard ending, Daniel fights Leo inside his own mind and destroys him. In the alternate ending, played as Leo, it is Leo who destroys Daniel's personality and takes control of his body. How the alternate ending is unlocked is not stated here, because it could not be confirmed.
The British Board of Film Classification refused to classify Manhunt 2 in June 2007 and rejected an edited version again. After appeals, the game was released in the UK with an 18 certificate on 31 October 2008. It was banned in Germany, New Zealand and South Korea. In North America it came out on 29 October 2007 on PlayStation 2, PSP and Wii.
