The main character of Manhunt is James Earl Cash, a death row inmate voiced by Stephen Wilfong. His enemy is Lionel Starkweather, "the Director", voiced by Brian Cox, who forces Cash to star in his snuff films. Cash fights through a series of gangs of "Hunters" before a journalist helps him turn the tables.

Lionel Starkweather, known as the Director, is the main antagonist of Manhunt (2003). A disgraced former film director who now makes snuff films, he fakes James Earl Cash's execution and directs him through an earpiece and CCTV cameras. Cash kills him in the final scene. Brian Cox voices him.
Once a successful director whose career collapsed, Starkweather uses his wealth to make snuff films. He controls Carcer City's corrupt police chief, Gary Schaffer, and employs private mercenaries called the Cerberus. His name likely comes from the spree killer Charles Starkweather. His film career is placed either in Los Santos or in Vinewood.

Cash kills Starkweather with Piggsy's chainsaw in Deliverance, the last scene. Starkweather begs for his life first. The journalist's evidence then exposes the ring and the police corruption. More on James Earl Cash and Piggsy.
Piggsy is the pig-masked, chainsaw-wielding man kept chained in the attic of Lionel Starkweather's estate in Manhunt (2003). A former actor in Starkweather's snuff films who lost his sanity, he is the boss of the final scene. His real name is never given. Hunter Platin voices him.
Piggsy breaks free and rampages through Starkweather's estate, which lets Cash escape the Cerberus. Starkweather then orders his mercenaries to kill both of them.
In Deliverance, Cash starts unarmed: stab Piggsy with the objects you find, such as wooden spikes and glass shards, until he runs off, then lure him onto a grate that gives way. Cash ends up with his chainsaw. The exact finish is not detailed here. More on James Earl Cash.
Only as an Easter egg: a Piggsy action figure in Zero's RC Shop in GTA San Andreas. On PC, the code PIGGSUIT unlocks his skin in Manhunt.
