The main characters of Max Payne 3 are Max Payne, voiced and performed by James McCaffrey; his friend Raul Passos (Julian Dean); the Branco family who employ them in São Paulo, Rodrigo, Victor and Marcelo, with Fabiana and her sister Giovanna; detective Wilson Da Silva (Stephen Girasuolo); and three enemies: Serrano's Comando Sombra gang, Álvaro Neves's Crachá Preto militia and Armando Becker's UFE police unit.

Nine years after Max Payne 2, Max has left the NYPD and is drinking heavily. Passos, whom he met during a run-in with the mob back home, brings him to São Paulo to protect the wealthy Branco family. Max's whole story is on the Max Payne character page.

Raul Passos is Max Payne's friend and fellow bodyguard in Max Payne 3 (2012). He brings Max to São Paulo to protect the Branco family, but is secretly on Victor Branco's payroll. Max forgives him in the end, and Passos leaves the city with Giovanna. Julian Dean voices him.

Passos meets Max during a run-in with the DeMarco mob family back in the United States, and offers him private security work for the Branco family in São Paulo.

Victor Branco is the main antagonist of Max Payne 3 (2012): a São Paulo politician and brother of Max's employer Rodrigo Branco. He hires Max as a fall guy, has Rodrigo killed and runs an organ trafficking ring with the corrupt UFE police unit. He is found dead in prison a week after his arrest. Robert Montano voices him.
Victor is the brother of Rodrigo and Marcelo Branco, and brother-in-law of Fabiana Branco and her sister Giovanna Taveres.

Victor flees to the airport. With detective Wilson Da Silva's help, Max destroys his private jet, and Victor survives. Da Silva convinces Max not to kill him, so Max breaks his leg instead.
A week later, Victor is found dead in prison.
Wilson Da Silva is a São Paulo police detective in Max Payne 3 (2012). Investigating the Crachá Preto militia, the UFE police unit and the city's disappearances, he becomes Max Payne's main ally and helps him bring down Victor Branco. Stephen Girasuolo voices him.
