The Ballad of Gay Tony has 26 story missions. Luis Lopez keeps his boss Tony Prince afloat while Tony owes money to the Ancelotti family and to Mori Kibbutz. A diamond deal ambushed by the Lost, a stolen batch of diamonds and a double-cross by Russian crime lord Ray Bulgarin lead to the final mission, Departure Time, where Luis kills Bulgarin on his plane and escapes by parachute.

The 26 missions below are listed below. Several run in parallel, so the order is not strictly chronological. Every mission is scored and can be replayed, and 100 percent on all of them unlocks the Gold Star achievement.

The episode opens after the Bank of Liberty robbery, where Luis was a hostage. Tony owes money to the Ancelotti family, through Rocco Pelosi, and to Mori Kibbutz. Luis settles the debt with Mori, helps his old friends Armando and Henrique, and steals an attack helicopter, an armoured personnel carrier and a subway car for billionaire Yusuf Amir.
Tony's $2 million diamond purchase is ambushed by Johnny Klebitz and the Lost, and Tony's boyfriend Evan Moss is killed. Luis gets the diamonds back by disrupting the museum deal between Johnny, Niko Bellic and the Jewish mob in Not So Fast. He goes to work for Ray Bulgarin, who then turns on him and reveals the diamonds were his.

Giovanni Ancelotti orders Tony and Luis to trade the diamonds for his kidnapped daughter Gracie, but Bulgarin disrupts the exchange and the diamonds are lost in the chaos. Rocco pushes Luis to kill Tony; Luis refuses and kills Rocco's accomplice Vince instead, and Bulgarin's men attack Tony's club. In Departure Time, with help from Yusuf, Luis goes after Bulgarin, kills him on his plane and escapes by parachute as a grenade destroys it. Yusuf then offers to get involved in the clubs, and in the final scene the diamonds are found by a homeless man.
