The Lost and Damned has 22 story missions. Johnny Klebitz, who kept the peace for The Lost MC while Billy Grey was in rehab, watches Billy break the truce with the Angels of Death and drag the club into a heroin deal gone wrong. Once Billy is arrested, Johnny becomes president, survives a civil war in the club and the diamond deals shared with GTA IV and The Ballad of Gay Tony, and executes Billy in the final mission, Get Lost.

Billy Grey Is Back
Billy Grey returns from rehab and breaks the truce Johnny had made with the Angels of Death. When new member Jason Michaels is killed, Billy blames the Angels, and the Lost raid their clubhouse and take a heroin stash. The heroin sale, set up by Elizabeta Torres with Niko Bellic and Playboy X, turns out to be a police bust, and Johnny escapes. He also starts working for congressman Thomas Stubbs III.
Johnny Becomes President
The heroin belongs to the Triads. At the deal to return it, Billy sets Johnny and Jim up to be killed, and Billy is arrested in the chaos in This Shit's Cursed. Johnny takes over the club, and Brian Jeremy leads a breakaway faction against him. The player can kill or spare Brian in Bad Standing. Johnny also kidnaps Roman Bellic for Dimitri Rascalov's men to pay off Ashley's debt.
The Diamonds
For Ray Boccino, the Lost ambush Tony Prince's diamond purchase, and Tony's boyfriend Evan Moss is killed. The sale of the diamonds at the museum, with Niko Bellic, is disrupted by Luis Lopez, but Johnny gets away with the money. Ray then kidnaps Johnny and Jim, who escape; Johnny fights off Ray's hitmen and learns from Ashley that Jim is dead.
How Does The Lost and Damned End?
Stubbs tells Johnny that Billy is about to testify against the Lost. In Get Lost, the club storms the Alderney State Correctional Facility and Johnny executes Billy. Back home, the clubhouse has been trashed by Boccino's men, and the Lost burn it down.
